Left Ventricular Angiocardiography by Transseptal Puncture of the Left Atrium.

چکیده
SELECTIVE angiocardiography of the left side of the heart is of prime importance for the accurate preoperative diagnosis of those lesions involving the mitral valve, ventricular septum, outflow tract of the left ventricle, or the aorta itself. The injection of contrast medium into the pulmonary artery for the demonstration of the left atrium and left ventricle is less satisfactory than the direct injection into those chambers. Until recently, left atrial and left ventricular angiocardiography has been achieved only by retrograde left ventricular catheterization or direct transthoracie puncture of the left atrium or left ventricle. Many reports deal with the advantages and disadvantages of these methods.1 5 The introduction of transseptal left heart catheterization6-9 was a major step forward in achieving safe access to the left side of the heart. Since its first description, the method was modified mainly for the purpose of left heart angiocardiography.10-13 There are several reports in the literature about the safety and reliability of transseptal left heart catheterizationl, 9, 10, 14, 15 whereas only a few studies on selective left heart angiocardiography by the transseptal route are available.1'0 12, 16, 17 Recently, Braunwald and co-workers17 published their experiences with 60 patients investigated with this new technic. It is the purpose of this paper to discuss the findings obtained in 90 patients in whom transseptal left heart angiocardiography has been performed.

Optimal Transseptal Puncture Location for Robot-Assisted Left Atrial Catheter Ablation
BACKGROUND The preferred method of treatment for atrial fibrillation (AF) is by catheter ablation, in which a catheter is guided into the left atrium through a transseptal puncture. However, the transseptal puncture constrains the catheter, thereby limiting its manoeuvrability and increasing the difficulty in reaching various locations in the left atrium. In this paper, we address the problem o...

Transseptal puncture (TSP) is a frequently performed procedure for gaining access to the left atrium for catheter ablation, hemodynamic assessment of the left heart, left ventricular assist device implantation, percutaneous left atrial appendage closure or mitral valvuloplasty during childhood and adulthood. The standard technique for transseptal puncture applies mechanical pressure on the foss...
متن کاملComparisons of mechanical versus phase-array intracardiac echocardiography-assisted transseptal puncture in patients with dilated left atrium undergoing percutaneous transvenous mitral commissurotomy.
BACKGROUND Percutaneous transvenous mitral commissurotomy (PTMC) is an effective treatment for mitral stenosis, but transseptal puncture carries a certain risk of complications. Our previous report has shown the effectiveness of phase-array intracardiac echocardiography (ICE)-guided transseptal puncture in patients with dilated left atrium undergoing PTMC. However, there are few reports compari...
متن کاملSite-Specific Transseptal Puncture for Emerging Structural Heart Interventions
• This article reviews the importance of precision in transseptal puncture to optimize left-sided structural heart procedures such as mitral valve (MV) repair, left atrial appendage (LAA) occlusion and mitral paravalvular leak closure. In addition, the article discusses the importance of guidance by various views on TEE and ICE and how these imaging modalities help guide site-specific transsept...

Intracardiac echocardiography (ICE) serves as an adjunct to fluoroscopy for electrophysiological procedures by identifying critical anatomic landmarks and confirming catheter-endocardial contact. In the present study, we investigated the usefulness of ICE for radiofrequency catheter ablation. ICE was utilized to guide transseptal puncture in 19 patients undergoing radiofrequency catheter ablati...
